Support Coordination within the National Disability Insurance Scheme (NDIS) is a service designed to assist participants in understanding and implementing their NDIS plans. A Support Coordinator works alongside participants to connect them with local service providers, build their capacity to manage their supports independently, and help them get the most value from their NDIS funding. Support Coordination is not included in every NDIS plan — it depends on individual circumstances and whether the NDIS determines it is necessary.
Short-term support to help participants understand their NDIS plan and connect with the right local services and providers.
Encourages participants to build their own support networks and strengthen their independence.
Helps participants design and implement a tailored mix of supports, including informal, community, and funded NDIS supports.
Supports participants to build their skills in managing services and living more independently.
Assists participants in resolving service issues and connecting with alternative providers when required.
For participants with more complex needs, such as high-risk situations or coordination across multiple service providers.
A Specialist Support Coordinator assists in managing complex challenges and ensures services are delivered appropriately and effectively.
Explain funding categories and help participants understand what’s available.

Support participants in reflecting on their progress and preparing for NDIS plan reviews to ensure continued support.
